Methods

The overall score was lower for pragmatic systematic reviews when the domains on the organization, flexibility of delivery and follow-up were merged.

It is important to remember that a pragmatic trial doesn't necessarily mean a low-quality trial, and in fact there is an increasing rate of clinical trials (as defined by MEDLINE search, but it is neither specific nor sensitive) that employ the term "pragmatic" in their abstract or title. The use of these terms in abstracts and titles could indicate a greater understanding of the importance of pragmatism, but it is unclear whether this is evident in the contents of the articles.

Conclusions

As the importance of evidence from the real world becomes more popular, pragmatic trials have gained traction in research. They are randomized trials that evaluate real-world alternatives to new treatments that are being developed. They are conducted with populations of patients more closely resembling those treated in regular medical care. This method can help overcome the limitations of observational research for example, the biases associated with the use of volunteers and the limited availability and coding variations in national registries.
